Strengths and difficulties questionnaire

The Strengths and Difficulties Questionnaire (SDQ) is a behavioural screening questionnaire about 3-16 year olds. It exists in several versions and languages and focuses on emotional symptoms, conduct problems, hyperactivity/inattention, peer relationship problems and prosocial behaviour. It can be completed by parents/carers, teachers and young people themselves.

The tool has a variety of uses, including to evaluate outcome via administering “before” and “after” an intervention. The questionnaire is available in multiple languages.

The 25 items in the SDQ comprise 5 scales of 5 items each. The scales include:

  • Emotional symptoms subscale
  • Conduct problems subscale
  • Hyperactivity/inattention subscale
  • Peer relationships problem subscale
  • Prosocial behaviour subscale

There is an extended versions of the SDQ called the impact supplement. This asks whether the respondent thinks the young person has a problem, and if so, enquires further about chronicity, distress, impairment in everyday activities, and burden to others.
